LIHEAP Application 2025-2026

Please answer the questions below to access the application for the LIHEAP program.

A valid email address is required to apply, as all decisions regarding your application will be sent via email.

Proof must be in the form of a Social Security Card or other documentation from the Social Security Administration. Please note that we cannot use Medicare cards or any other secondary documentation for SSNs.

You must provide one of the following valid and non-expired IDs:

  • State issued ID’s

  • Birth Certificates

  • Passports

  • Military ID’s

  • Voter’s Registration Cards

This can included all of the following:

  • Wages: Weekly Pay - Submit 4-5 check stubs.  Bi-Weekly Pay - Submit 2-3 check stubs.

  • Social Security/Disability (SSI): Submit the current year's award letter (showing net amount) OR a current bank statement showing the deposited amount.

  • VA and Pension: Submit the current year's award letters, showing gross amount. (Year-end tax forms are NOT acceptable proof of income.)

  • Self-Employment: Submit tax returns OR gross amounts after business costs (ledgers/receipts).

  • Other Income (if applicable): Provide documentation for Child Support, Alimony, or Unemployment.

You must provide a copy of one of the following:

  • DD-214

  • VA ID.

You must provide a one-year rent ledger from your site office.
